We accept many insurance plans

Accepted plans include:

  • Blue Cross/Blue Shield
  • Medicare
  • Railroad Medicare
  • Northwest Community Health Partners (NAMM)
  • Health Plus Physicians Organization (HPPO)
  • Workers' Compensation

If Dr. Foroohar recommends a procedure that requires pre-approval for benefits under your insurance plan, our office staff will help you submit the necessary paperwork to satisfy your plan's pre-certification requirements. Dr. Foroohar's office will obtain the pre-certification for imaging including MRI, as well as surgery.

What if you have a different insurance plan?

“We try to work with all patients, even if we are not in network with their plans,” says Dr. Foroohar. “Most insurance plans have out-of-network benefits. My office staff will work with you and your insurance company to help determine the benefits your plan will pay.

“Since there are relatively few neurosurgeons, I do evaluate and treat many patients with out-of-network plans,” she adds.

If you belong to an HMO, do you need a referral?

Yes. If your insurance is through the NAMM or HPPO HMO program, please make sure you have the referral your plan requires before each appointment.

What if your case is covered by Workers' Compensation?

Dr. Foroohar's office will coordinate with the Work Comp adjustor and case manager to arrange for your appointment. All Work Comp patients need pre-approval and authorization before being seen.

Does Dr. Foroohar see patients with auto accident insurance?

Yes. Dr. Foroohar treats patients covered by auto accident insurance, but only if it is the patient's own insurance, not a third party's policy. As with Work Comp cases, we will need to verify auto accident insurance benefits before the patient is seen.

Answers to your questions about payment

When is payment due?

Payment, or your insurance co-pay, is expected on the date of service. We accept cash, checks and major credit cards.

When is payment due for surgery?

Dr. Foroohar's office will submit surgery claims to your insurance carrier. You are expected to pay any balance not paid by your insurance in 30 to 90 days.
